Pre K Applications

Pre K is for children between 3 and 4 years old.  There are either morning or afternoon half day class, which are Monday to Thursday.  All applications will be reviewed by the teacher and they will contact you when spots become available. Download, complete and return the following application.

If your child(ren) is not Catholic, please fill out the following Non-Catholic Declaration Form:

Please also provide your child(ren)'s birth certificate as well and a baptismal certificate if your child is Catholic.
